In "Without A Paddle" there's a scene where Matthew Lillard's character is talking to his girlfriend on the phone, and Dax Sheppard and Seth Green get their cue wrong and step into the shot too early.

After just watching "Airplane" here are some slip-ups I noticed (well, actually, they're pointed out by the filmakers' in the commentary).
1. During the Saturday Night Fever scene, Robert Hayes can be seen in the background waiting for his cue to step into the scene, after his stunt double does a backflip into the audience.
2. A crewmember can be seen at the left hand side of the screen when Ted Striker (Hayes), chases down Elaine (Julie Haggerty).
Mistake I always noticed in "Back to the Future II". When marty is being chased by Biff's minions, and Marty is running up the circualr staircase, you can see Marty run past a crew member. You can't see the crew guy's body, but you definetely see his shadow on the floor. Basically, the crew guy is there to give Marty his cue to run up the staircase, after Biff's minions pass by.

While the movie Resident Evil is kind of a goof in and of itself, there are a couple scenes where you can see the microphone dangling down in front of the actors. It's somewhat close to the laser-room scene where nearly everyone dies.
Also, the lamp Peter Parker destroys in Spider Man briefly returns to life in the next scene.

I don't care what anyone says, the absolute worst goof I have ever seen is in The Doors. At one point Meg Ryan calls Val Kilmer "Val" during a scene. I still don't understand how that got through the editing room. There are two good ones in Terminator 2 also. The first is the cigar burn that appears and disappears a few times. The second is when the truck jumps the bridge and you clearly see all of the windows smash out. In the very next scene they are magically repaired!

My favourite is in "Commando", the Arnie classic. The crashed Porsche is wrecked when Arnie flips it back the right way, but when they drive off it's miraculously repaired.

In Teenage Werewolf, an extra exposes himself in one scene, and it made the final cut!
